Students ordering tests for research must prepay and have
their supervising faculty member sign the form.
Certain tests published by CPP are only available to users who have appropriate training and credentials, and who adhere to the principals of proper test use, including knowledge of tests and their use.
To receive "B" materials, you must have a degree from an accredited college or university and have satisfactorily completed a course in the interpretation of psychological tests and measurement at an accredited college or university. To receive "C" materials, you must fulfill all qualifications required of "B" users, plus possess an advanced degree in a profession that provides training in the interpretation of psychological tests.
The difference between "B" and "C" level instruments lies in the level of training needed for their interpretation.
Completion of this form is not required of licensees ordering for a regionally accredited school, college, or university nor for a federal or state agency if, and only if, an official purchase order is used and payment is made by institutional voucher or check. All licensees must sign this form. Subagencies should include documentation of their affiliation with a qualified institution.
Licensing of Test Instruments; Resale and Export Prohibited:
CPP licenses but does not sell copies
of its test instruments; all such licenses are limited to
the personal use of the licensee only. Certain of
CPP's test instruments may be licensed only by
professionals whose qualifications are on file with and
accepted by CPP. No copy of any CPP-licensed test
instrument may be resold, sublicensed, exported,
redistributed or otherwise transferred; nor may any such
copy be used in any manner by any party other than the
person or entity to whom that copy is licensed for use by
CPP; any violation of any of these restrictions shall
automatically terminate the license to use the test
instrument(s) in question.
